Year 9 Immunisations - Consent Form
Dear Parents/Carers of Year 9 students,
Your child is now due for their Td/IPV booster and Meningitis ACWY vaccination.
You are reminded to please complete the following form to state whether you wish for your child to have the vaccines or not:

Even if you do not wish for your child to have the vaccines, please complete the online form so that we can record your wishes.
